The two-year lookback

Your premiums this year are based on the MAGI from your tax return two years ago. So income you create now โ€” including a Roth conversion or a big capital gain โ€” can raise your Medicare premiums about two years later.

People near a threshold often watch their income late in the year, because a one-time spike โ€” a large withdrawal, a property sale, a year-end bonus โ€” can push them over a line. This tool is here to show where those lines are, not to tell you what to do.

A life-changing event may let you appeal

Retirement, the death of a spouse, marriage, or divorce can let you ask Social Security to use more recent income via Form SSA-44. This tool doesn't handle appeals.

After a spouse passes, costs can rise

The survivor usually files as Single, where the IRMAA thresholds are lower โ€” so the same income can land in a higher tier and cost more.
